To maintain the certification of your therapy dog the following vaccinations and health requirements must be kept current using the attached form. The paperwork below will need to be updated and signed by the licensed veterinarian who oversees care for the dog. 

 

Required vaccinations and exams include:

• Rabies - must be given by a veterinarian.

• DHHP - Distemper, Hepatitis, Parainfluenza and Parvovirus. 

• Annual health exam

• Annual fecal test

• Annual heart worm test
